Aw, geez. Did the vet check her eyes? I wonder if there's something going on with her vision. I hate to say the brain tumor words, but it sounds like that could be another thing that needs to be ruled out.
It does sound like she's safest in her crate, and you could cover it if that helps her calm down. Be careful, too...in this state she could bite you without meaning to just out of sheer terror.
