By making the apple slices bigger than the opening, is the object for them NOT to get the slices out?

And just so I understand...Do I put the peanut butter in the opening first and then the apple slices or the other way around? Pardon my ignorance here. I was just picturing setting the Kong on the counter to insert this yummy treats at which time the opening is at the top, so the rounded closed end is at the bottom . Which is Which?

What size? Do I purchase the size that is reccomended by the manufacturer for the size/weight of the dogs I have? When I buy knuckle bones for the girls, even though Greta's muzzle and jaw opening is slightly bigger than Fozzy, I have to get a knuckle slightly smaller for Greta .
