When a pup is teething is typically when you see the most ear set problems occurring....typically by 6-6.5 months most ear problems fall into line. My vet has always encouraged me to massage the cartilege in their ears while teething and it has always worked for my personal dogs for correcting some of the ear problems caused by teething.
I have taped (literally) ears down to the side of their head to promote a good ear set in pups however.
The only reason I commented to the OP that the ears may fly out again is that his pup is only 21 weeks (ie 5 months/1 week) she has a LOT of teething yet to do!!! After teething is over typically ear sets at least in my experience maintain fairly normally.
