That would be my first suggestions as her behavior may be caused by various health coniditons: yeast infection, UTI, incontinence (was she spayed recently?), bladder infection, giardia, other intestinal parasites, bacteria, viruses, etc. to name a few...
If all her tests are clear, go back to square one with potty training. When in house - she's in her crate or leashed. Potty breaks are at regular intervals, regardless of "her schedule". Start at every 30 minutes, then escalate to every hour, etc. High praise (maybe even a treat) for doing it right, no yelling or scolding (a firm "no" if caught in the act) for doing it wrong.
Edited to add: And clean the accident areas with enzyme cleaner ("Nature's Miracle", etc.) to get the smell out.
