I include all of the above, including copies of the vet records (i.e. well puppy exam before being sent home). I do not vaccinate before puppies leave, but if the new owners are comfortable giving their own vaccines, I will provide the Proguard DPV.

If the dogs are breed tested, copies of that goes in too. I also put in articles that are important to me (i.e. supporting data for the recommended vaccine schedule) in terms of the pup's future health. Sometimes it's easier for the new owners to "discuss" things with their vet if they have valid cites with them, instead of just "my breeder said so." My next crew will get a copy of the Purdue study regarding later neutering and decreased risk of osteo.

I also have the new owners drop a crate off several weeks before the puppy is to go home, the crates are labelled and the puppies practice "crate time" in their own crates. This way, it also leaves with some of our home's smells in it.
