I think it is fine to ask for an extra crate. The puppy will be used to his/her own crate which will be a plus when the puppy does get to his/her new home. But to ask for money to cover food and shots is petty IMO. If I was that puppy buyer, I would be seriously thinking second thoughts about having a relationship with you as the breeder.
Consider it the cost of doing business.
