I would establish whether she was pregnant or not. If she was pregnant I would spay her.
You sound like you are/were a responsible owner so don't be to hard on yourself. If she isn't pregnant then your precautions DID work and anything you question yourself on now can be remedied in the future. Here's hoping it's a falsie. lol
