you really should not "bathe" a pup until it is at least 6 wks of age. Rinse, maybe. Most soaps and shampoos are too harsh for a young pups skin and could cause these dry spots and hair falling out. Puppies are not going to stay out of food, they are going to crawl around in it and get it all over the place, but the mother and siblings usually tend to take care of cleaning it up. She should take the pups to the vet and have it checked out though. It may be something other than the soap.
